Practice desciption
When is the last time you really took the time to learn from failures (or successes)? Without honest evaluation, making the same mistakes is inevitable. We call this a post-mortum analysis.

What steps to take for the practice
- Organize a post-mortum session.
- Make sure to schedule it as soon as possible. So that everybody know this session is coming up after the project.
- Also, don't forget to create a clear schedule for the session.
- Discuss “what worked” and “what did not work” with your team.
- Take notes
- Keep it light
- Don't make it personal
- Respect everyone's opinion
The follow up is almost as important as the session itself. Address the action points at the end of the meeting and agree on who takes what action.
